The Community Health Worker (CHW) position will support the implementation of strategies to improve access to and availability of healthy foods and beverages within the community. Responsibilities include sorting and distributing produce for community distributions, contributing to program marketing efforts and event promotion, and engaging with parents, teachers, and community members by sharing produce and available resources. The Community Health Worker will identify and collaborate with internal and external organizations to connect community members with a variety of resources and services. Additionally, the CHW will assist individuals with applying for CalFresh benefits by helping them navigate the application process and submit required documentation.
Other program-related duties may include administrative tasks such as developing promotional materials and collecting and analyzing data to identify community needs and inform program planning.
Essential Job Responsibilities
1. Conduct culturally appropriate nutrition and health education activities and develop supporting educational materials as needed.
2. Document program activities and maintain records of participant engagement, survey results, and community reach.
3. Develop and maintain cooperative working relationships with supervisory staff, community partners, external collaborators, parents, and TCCI families.
4. Serve as the primary point of contact for childcare sites regarding program activities, including produce distribution sign-ups and coordination.
5. Prepare and distribute health education materials, including reports, bulletins, handouts, and visual aids such as videos, and photos
6. Collaborate with partners and stakeholders to assess community health needs and available services in order to develop strategies and goals to address identified needs.
7. Maintain databases, mailing lists, communication networks, and other information systems necessary to support program operations.
8. Complete, organize, and maintain all required program documentation, records, and reports in accordance with program requirements.
9. Coordinate and participate in outreach activities as needed, including community fairs, health events, presentations, and resource-sharing opportunities with community groups and agencies.
10. Assist with the planning, preparation, sorting, and distribution of produce for community food access events and distributions.
11. Support program marketing efforts, including the development and dissemination of promotional materials and community outreach messaging.
12. Assist community members with CalFresh applications by providing education, referrals, and navigation support throughout the enrollment process.
Required Qualifications
1. Bachelor’s Degree in Health, Health Care Studies, Social Services or related field- Preferred
2. Prior experience implementing health promotion, nutrition education, community engagement events, and networking skills.
3. Ability to work with families and individuals as a representative of the agency
4. Excellent interpersonal written and verbal communications skills
5. Computer literate
6. Experience working with low-income, minority and inner-city populations

Pay: $24.50 - $26.50 per hour
