Position Overview
We are seeking a compassionate, dependable, and highly organized Medical Care Coordinator & Senior Caregiver to provide weekday, in-home support for two elderly family members in their 90s. This role is ideal for someone with hands-on eldercare experience, strong judgment, patience, and the ability to coordinate care across family members, nurses, physicians, and other providers.
The gentleman requires daily assistance due to significant vision impairment, use of a walker, mobility needs, blood pressure monitoring, medication administration, meal support, and transportation or accompaniment to medical appointments as needed. The woman is physically healthy but is showing signs of early dementia and requires attentive supervision, redirection, companionship, and safety oversight during the day.
This person will serve as the primary Monday through Friday daytime caregiver and care coordinator, ensuring both individuals remain safe, comfortable, nourished, supported, and well cared for while keeping the family informed and aligned with the broader care team.
Key Responsibilities
Daily Care & Mobility Support
- Provide hands-on daily support for an elderly gentleman who is almost fully blind and uses a walker.
- Assist with safe movement throughout the home, fall prevention, transfers, and mobility-related needs.
- Monitor physical comfort, safety, balance, and overall wellbeing throughout the day.
- Ensure the home environment remains organized, safe, and accessible for both seniors.
- Provide calm, patient, and respectful support while promoting dignity and independence whenever possible.
Dementia Awareness, Supervision & Companionship
- Provide attentive supervision and companionship for an elderly woman showing signs of early dementia.
- Monitor activity to help ensure she remains safe and does not engage in unsafe or risky behavior.
- Use patience, reassurance, and respectful redirection when confusion, forgetfulness, repetitive questions, or disorientation arise.
- Maintain consistent daily routines that support comfort, stability, and emotional wellbeing.
- Communicate notable changes in mood, memory, behavior, or safety concerns to the family.
Medication Administration & Health Monitoring
- Administer medications to the gentleman as directed and maintain accurate medication records.
- Monitor and record blood pressure readings several times per day, or as directed by the family or physician.
- Track medication timing, refills, physician instructions, and any observed side effects or concerns.
- Escalate urgent health concerns, missed medications, abnormal readings, or changes in condition promptly to the family and appropriate care team members.
- Maintain organized care notes for continuity with night and weekend nurses.
Meal Preparation & Daily Household Support
- Prepare and serve breakfast and lunch each weekday. This does not require extensive cooking.
- Support hydration, nutrition, and dietary needs based on family or physician guidance.
- Monitor food, household, medication, and care supply needs and communicate when items need to be replenished.
Appointment Coordination & Transportation
- Manage and coordinate approximately four medical appointments per month, including routine doctor visits, specialist appointments, and follow-up care.
- Maintain an organized calendar for appointments, medication routines, care coverage, and related reminders.
- Provide or coordinate transportation to and from medical appointments as needed.
- Accompany seniors to appointments when appropriate, take notes, and communicate physician instructions and follow-up items to the family.
- Confirm appointment details, preparation instructions, and transportation plans in advance.
Care Coordination & Family Communication
- Serve as the primary weekday point of contact between the family, weekend/night nurses, physicians, specialists, pharmacies, and other care providers.
- Provide clear, timely, and professional updates to authorized family members regarding daily care, health observations, appointments, medication needs, and concerns.
- Coordinate with existing weekend and overnight nursing staff to ensure smooth handoffs and consistent care.
- Maintain accurate daily notes and communicate important changes or follow-up needs.
- Protect the privacy and dignity of both seniors and maintain confidentiality at all times.
Qualifications & Skills
Required Experience
- Minimum of 2 to 3 years of experience in private caregiving, home health, eldercare, assisted living, nursing support, or a similar hands-on care role.
- Direct experience assisting seniors with mobility limitations, walker use, fall prevention, medication routines, and daily living support.
- Experience supporting individuals with dementia, memory loss, confusion, repetitive questioning, or cognitive decline strongly preferred.
- Comfort preparing simple meals, maintaining routines, and providing respectful daily companionship.
Preferred Certifications
- Certified Nursing Assistant (CNA), Home Health Aide (HHA), Medical Assistant (MA), Licensed Practical Nurse (LPN), or comparable experience preferred.
- CPR and First Aid certification preferred.
- Valid driver’s license, reliable transportation, and a clean driving record preferred if transportation support is required.

Role Expectations
Schedule
Daily weekday coverage, Monday through Friday. Coordination with existing night and weekend nurses is required.
