What you'll do
You'll provide direct, one-on-one care in patients' homes as part of a multidisciplinary team that includes RNs, LPNs, PT, OT, speech therapy, social work, and home health aides. Responsibilities include:
- Complete clinical assessments and develop/update plans of care
- Keep physicians informed of patient conditions and report any changes
- Perform venipuncture and wound dressing changes as necessary
- Supervise care provided by home health aides
- Collaborate with your team to ensure comprehensive, coordinated care

Qualifications
- Valid Pennsylvania Registered Nurse license
- Valid driver's license
- Previous experience as an RN
- Strong clinical assessment skills
