Responsibilities
- Deliver direct patient care by monitoring vital signs, administering medications, and performing basic medical procedures such as catheter care and tube feeding.
- Utilize Electronic Medical Record (EMR) and Electronic Health Record (EHR) systems to accurately document patient information, medical records, and care activities in compliance with HIPAA regulations.
- Assist with activities of daily living including mobility support, hygiene assistance, and memory care for individuals with dementia or Alzheimer’s.
- Support nursing staff by preparing patients for exams, updating medical documentation, and managing medical supplies.
- Conduct assessments of physiological functions and respond promptly to changes in patient conditions or emergencies.
- Work closely with interdisciplinary teams to coordinate home & community care services, hospice support, or acute care interventions as needed.
- Maintain a safe environment by adhering to infection control protocols and safety standards related to assisted living, hospital settings, or working with individuals with disabilities.
Qualifications
- Prior experience in assisted living facilities, nursing homes, hospitals, or home care environments is highly preferred.
- Knowledge of medical terminology, anatomy, physiology, and medication administration practices.
- Experience working with individuals with developmental disabilities or those requiring specialized memory or dementia care.
- Proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with medical documentation standards including HIPAA compliance.
- Strong caregiving skills combined with the ability to perform clinical tasks like vital signs measurement, catheter care, and tube feeding management.
- Excellent communication skills to collaborate effectively with healthcare teams and family members.
- Valid certifications related to nursing assistance or medical technology are advantageous; ongoing training in hospice or acute care is a plus.
