Job Title: Licensed Clinical Social Worker (LCSW)
Location: Seattle, WA 98122
Schedule: Day Shift – 5x8 Hours (08:00 AM – 04:30 PM)
Duration: 13 Weeks
Compensation:
- FROM $1,800 per week (Local Candidates)
- FROM $2,200 per week (Traveler Candidates)
Key Responsibilities:
- Conduct psychosocial assessments and develop care plans for acute care patients.
- Coordinate discharge planning, including arranging community resources and post-acute care services.
- Provide crisis intervention, case management, and collaborate with interdisciplinary teams.
- Assist with advance care planning, protective services, and patient advocacy.
- Maintain accurate and comprehensive documentation.
- Ensure continuity of care through referrals and appropriate resource linkage.
Required Licenses & Certifications:
- Active state licensure (LSWAIC or equivalent)
- BLS certification (AHA)
Education & Experience:
- Master’s Degree in Social Work or related field (required)
- Minimum 1 year of recent inpatient case management experience
