Expected Annual Earnings: $107,000 - $120,000+
$30,000 Sign-On Bonus

One Territory would be Northside (Oceanside, La Jolla, Carlsbad)
The other would be Southside (Chula Vista, Lemon Grove, National City)
Productivity Expectation: 30 Points Weekly
Mileage Reimbursement: $.725 Per Mile Between Visits

Territory
This position serves patients throughout the San Diego market and surrounding communities. Patient schedules are designed to be geographically efficient, and clinicians are not usually expected to cover the entire territory in a single day.
Essential Responsibilities
- Provides appropriate clinical management of the patients assigned to the schedule. Conduct comprehensive OASIS assessments and care plan development.
- Provide skilled nursing care as outlined in the plan of care. Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
- Practice knowledge of Medicare CoP's, proficient documentation skills, attention to detail, prioritization, and time management.
- Evaluate effectiveness of services provided to the patient, consulting with physicians as needed.
- Submit detailed clinical documentation within expected timeframe.
- Monitor clients' conditions; reporting changes to Clinical Manager and Physicians. Follow up with, executing, and properly documenting doctors' orders.
- Case management and care coordination while in the field with support from the clinical manager.
- Assure that care is in accordance with Federal and State guidelines.
- Full time minimum visit expectation per established metrics as defined in offer letter.
- Perform on call rotation as assigned per on-call schedule.
Qualifications
- Registered nurse with current licensure to practice professional nursing in the State of California. Demonstrated ability to supervise and direct professional administrative personnel
- Experience in case management, disease management, home health care nursing, hospital nursing or intensive out patient education.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Pre-Employment Requirements
- Current Tuberculosis (TB) Test or Chest X-ray
- Titers or proof of immunity for Varicella, MMR, and Hepatitis B
- Proof of Tdap vaccination
- Proof of annual Influenza vaccination
- Current physical examination
