Position Title: Licensed Vocational Nurse (LVN)

Position Summary
Seeking a dedicated and experienced Licensed Vocational Nurse (LVN) to provide skilled nursing services to patients in their homes. The ideal candidate will have experience in wound care management, Wound VAC therapy, IV therapy, medication administration, and chronic disease management. This position requires strong clinical judgment, excellent communication skills, and the ability to work independently in a home health setting.
Essential Duties and Responsibilities
Perform skilled nursing visits in accordance with physician orders and agency policies.
Provide comprehensive wound assessments and wound care treatments.
Manage and monitor Wound VAC (Negative Pressure Wound Therapy) systems.
Perform IV therapy, including IV medication administration, PICC line care, and infusion monitoring.
Administer medications and treatments as prescribed. Monitor and document patient condition, progress, and response to treatment.
Educate patients and caregivers regarding disease processes, medications, wound care, and safety measures.
Identify changes in patient condition and promptly report findings to the supervising RN and physician.
Assist in preventing avoidable hospitalizations through early intervention and patient education.
Maintain accurate, timely, and complete clinical documentation. Participate in interdisciplinary care planning and case conferences.
Ensure compliance with state regulations, agency policies, and infection control standards.
Qualifications Required:
Current Texas Licensed Vocational Nurse (LVN) license in good standing.
Minimum of one (1) year of nursing experience.
Experience with wound care and wound management. Experience with Wound VAC therapy.
Experience with IV therapy and infusion services.
Current CPR/BLS certification.
Reliable transportation and valid Texas driver's license. Ability to travel throughout assigned service areas.
Preferred: Home Health experience.
PICC line and central line management experience. Tracheostomy care experience.
G-Tube/Enteral Feeding experience.
Pediatric and adult skilled nursing experience.
Bilingual (English/Spanish).
Skills and Competencies
Strong clinical assessment skills.
Knowledge of wound healing principles and infection prevention.
Ability to manage multiple patient assignments independently.
Excellent written and verbal communication skills.

Competitive Pay: $28.00 – $35.00 per hour (based on experience)
