Position Overview
The NFT Navigator helps Medicaid residents in nursing facilities move back into the community. They create support plans, connect people with resources, and provide ongoing support to help them stay independent.
Responsibilities
- Help nursing facility residents transition to community living
- Complete assessments and create support plans
- Coordinate services and connect people with resources
- Do outreach to facilities and residents
- Keep documentation up to date and complete reports
- Stay in contact with clients (calls and in-person visits)
- Meet monthly goals and expectations
- Travel regularly within the service area
Required Competencies
- Knowledge of HIPAA and Mandated Reporter guidelines.
- Familiarity with person-centered processes.
- Knowledge of developmental and mental health disabilities.
- Strong organizational, time management, and problem-solving skills.
- Excellent written and verbal communication.
- Ability to work independently, maintain confidentiality, and respect consumer choice.
- Comfort with public speaking and community outreach.
- Computer proficiency and ability to track timelines.
Qualifications
Candidates must meet ONE of the following:
- Licensed Registered Nurse in Michigan, OR
- Licensed Social Worker in Michigan, OR
- Bachelor’s degree in a health or human services field, or Community Health Worker certification and at least 3 years of experience providing health or social services.
Additional Requirements:
- Understanding of the Independent Living Philosophy.
- Three or more years of experience working with community resources.
- Ability to travel to nursing facilities, consumer homes, and community sites, including locations with stairs.
Work Expectations
- Maintain a caseload of 12–20 consumers.
- Cover assigned nursing facilities monthly.
- Ensure all documentation is completed within 24 hours.
- Attend agency meetings, trainings, and collaborative community projects.
- Work a full 20–30-hour week, with occasional flexibility outside typical business hours.
Work Environment & Travel
Work occurs in office settings, consumer homes, community locations, and nursing facilities. Regular local travel and some out-of-area travel are required.
