Job Summary
We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in various settings, including nursing homes and clinics. This role requires a strong understanding of patient needs, effective communication skills, and the ability to work collaboratively with healthcare professionals to ensure optimal patient outcomes.
Responsibilities
- Administer medications and treatments as prescribed by physicians.
- Monitor and record patient vital signs and health status.
- Provide direct patient care, including assistance with daily living activities.
- Implement aseptic techniques to maintain a safe and sterile environment.
- Conduct health coaching sessions to educate patients on wellness and disease prevention.
- Assist in family planning consultations and support for patients.
- Deliver specialized care for patients with dementia and behavioral health issues.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Utilize Cerner or similar electronic health record systems for accurate documentation.
- Perform triage duties to assess patient needs effectively.
Requirements
- Valid Licensed Vocational Nurse (LVN) license in the state of practice.
- Strong knowledge of physiology and nursing principles.
- Experience in infant care is a plus but not mandatory.
- Previous experience in nursing homes or similar healthcare environments preferred.
- Familiarity with behavioral health practices is advantageous.
- Excellent communication skills, both verbal and written.
- Ability to work effectively within a team-oriented environment.
