We're looking for an experienced Registered Nurse to join our clinical leadership team as our next MDS Coordinator. ✔ Weekend on-call rotation as assigned
$90,000-$100,000 annually

Coordinate and complete all MDS assessments and resident care plans
Ensure timely, accurate documentation and regulatory compliance
Lead interdisciplinary care plan meetings with residents and families
Monitor CMS Quality Measures and identify opportunities for improvement
Collaborate with nursing, therapy, dietary, social services, and rehabilitation teams
Support Medicare reimbursement accuracy through complete documentation
Participate in QAPI initiatives and survey readiness
Assist with policy development, clinical documentation, and quality improvement initiatives
Help maintain compliance with federal and state regulations
Active Registered Nurse (RN) license in Maryland
Proficiency with electronic medical records
CPR/First Aid certification (or ability to obtain)
MDS experience required
Strong knowledge of MDS regulations and submission process
Experience in long-term care or skilled nursing preferred
Strong organizational, communication, and leadership skills
