We are seeking an experienced, highly organized, and compassionate Hospice Director of Nursing (DON) to oversee the clinical operations of our hospice agency. This leadership role requires a licensed Registered Nurse with hospice experience, strong knowledge of Medicare and state regulations, and the ability to manage nursing staff, coordinate patient care, and ensure compliance with hospice standards. The ideal candidate will be professional, detail-oriented, and comfortable leading an interdisciplinary team while providing compassionate support to patients and families during end-of-life care.
Duties
Oversee daily clinical operations for the hospice agency and ensure quality patient care is delivered according to hospice standards.
Supervise and support nurses, LVNs, CHHAs, social workers, chaplains, and other interdisciplinary team members.
Coordinate patient admissions, recertifications, discharges, and ongoing care planning.
Review clinical documentation to ensure accuracy, timeliness, and compliance with Medicare, state, and accreditation requirements.
Participate in and help lead IDG/IDT meetings to review patient status, care plans, and service needs.
Work closely with the Medical Director, Administrator, and clinical team to ensure proper coordination of care.
Monitor patient visits, staff schedules, and care delivery to ensure patients and families receive timely support.
Provide clinical guidance, training, and support to staff members as needed.
Assist with quality assurance, performance improvement, survey readiness, and regulatory compliance.
Help resolve patient, family, caregiver, and staff concerns in a professional and compassionate manner.
Ensure hospice policies and procedures are followed by the clinical team.
Maintain confidentiality and professionalism when handling patient records and sensitive information.
Experience
Current California Registered Nurse (RN) license required.
Hospice experience required.
Previous experience as a Hospice DON, Clinical Supervisor, Case Manager, or nursing leadership role is preferred.
Strong understanding of hospice care, Medicare requirements, plan of care documentation, recertifications, and compliance standards.
Experience supervising clinical staff and coordinating interdisciplinary care.
Excellent communication, leadership, organization, and problem-solving skills.
Ability to manage multiple priorities while maintaining attention to detail.
Experience with hospice surveys, audits, ADRs, or accreditation preparation is preferred.
Knowledge of hospice EMR systems is preferred.
Valid driver’s license and reliable transportation preferred.
