Reports to: Youthbuild Program Manager
Schedule: Full-time 40 hours/week
Pay Rate: $45,000 - $50,000, salaried position

Summary
The Youthbuild Case Manager works closely with the Youthbuild Program Manager and Youthbuild team to support members in their professional development, exposure to apprenticeship opportunities, credential progress, and long-term career readiness. This position owns and tracks the individualized support, development, documentation, and next-step planning for each assigned Youthbuild member.
This role requires strong follow-through, clear communication, accurate case notes, and the ability to keep members moving toward program completion, credential attainment, placement readiness, and long-term stability. This position is not only resource coordination. It also requires member engagement, accountability, mentorship, and documentation on a regular basis.
Youthbuild is a national model that teaches trade skills to Milwaukee residents ages 18-24. Through education in construction and trades, we empower young people to follow a career, pursue their passions and thrive with the knowledge they need to support themselves, their families and community. The Youthbuild Case Manager will be an integral part of the program's workforce development goals, including building relationships with companies, business reps, unions, and partners to maintain pipeline leads and assist with member placement preparation.
Essential Duties and Responsibilities
- Assess needs and coordinate resources: Evaluate member needs and assist with resources, referrals, barrier removal, and problem-solving.
- Develop individualized member plans: Create and maintain plans that help members achieve their own goals and the goals of the Youthbuild program. Plans must be updated at the 6-month mark, exit, and any time major goals, barriers, or next steps change.
- Own each assigned member's plan: Track goals, barriers, next steps, deadlines, referrals, credential progress, placement readiness, and follow-up needs.
- Help members understand their own plan, deadlines, and next steps so they are active participants in their progress.
- Professional development: Assist members with interview coaching, resume building, apprenticeship exposure, job readiness, and career pathway preparation.
- Coordinate with instructors, site supervisors, partner case managers, workforce partners, unions, contractors, and other internal staff to maintain a shared understanding of each member's progress and needs.
- Work with partner case managers to confirm how each organization is serving the member, what resources are being provided, and what follow-up is still needed.
- Maintain records in accordance with grant requirements set by funding sources and the organization.
- Maintain complete, detailed, timely, and useful case notes.
- Proactively communicate member updates, concerns, missed deadlines, and follow-up needs to the Youthbuild Program Manager and relevant team members.
- Take ownership of assigned projects, member follow-up, and documentation without needing repeated reminders.
- Uphold the values and mission of Youthbuild, Employ Milwaukee, and Milwaukee Community Crossroads.
Documentation and Compliance
The Youthbuild Case Manager is expected to maintain documentation that is accurate, timely, and usable by the full team. Case notes should not only confirm that a conversation happened. They should show what was discussed, what action is needed, who owns the next step, and when follow-up will happen.
- Maintain case notes that reflect meaningful engagement, barriers identified, resources provided, action steps assigned, and follow-up dates.
- Track member progress against Youthbuild requirements, including attendance, credential goals, professional development, career readiness, apprenticeship exposure, support service needs, and exit planning.
- Ensure documentation supports internal accountability, grant compliance, partner coordination, and member progress tracking.
- Flag missing documentation, missed deadlines, or member concerns early so the team can respond before issues grow.
Mentorship and Member Engagement
This role requires a mentorship mindset. The Case Manager should build trust with young adults while helping them take ownership of their choices, goals, responsibilities, and next steps. The right candidate will balance encouragement with accountability and will see each interaction as part of helping members build long-term personal and professional stability.
- Build consistent, respectful relationships with Youthbuild members.
- Use coaching and mentoring conversations to help members connect daily choices to long-term goals.
- Balance support with accountability when members miss appointments, fall behind, or struggle to follow through.
- Maintain clear professional boundaries while staying invested in member success.
Performance Expectations
Within the first 90 days, success in this role should be visible through the following:
- Case notes are current, specific, and useful.
- Each active member has a clear plan with next steps and deadlines.
- The Youthbuild Program Manager does not have to chase basic follow-up.
- Communication is proactive when a member is off track.
- Referrals and resources are tracked through completion, not just mentioned.
- The Case Manager can clearly explain where each member stands.
- Members know what they are working toward.
- Staff trust the information being entered and shared.
Qualifications
- Experience in case management, especially with young adults, workforce development, or populations facing barriers to employment and education.
- Strong documentation skills, including clear case notes, tracking systems, and follow-up records.
- Proficiency in computer skills, including Excel and data tracking tools.
- Strong written and verbal communication skills.
- Ability to independently manage projects, deadlines, follow-up tasks, and member plans.
- Ability to work as part of a team and communicate clearly with internal staff and external partners.
- Valid driver's license and insured vehicle for program-related travel.
- Bilingual Spanish/English skills preferred but not required.
- Experience with nonprofits and/or government-funded programs preferred.
Work Environment
Milwaukee Community Crossroads is a multi-service organization that works to build strong neighborhoods full of opportunity. We approach that work through four core areas: housing, youth development, older adults, and food security – serving over 7,500 people each year. What began with American Baptist roots providing outreach to immigrants on Milwaukee’s south side in 1921, MCC now serves youth, families, and older adults of all races, ethnicities, religions, and ages throughout the city of Milwaukee.
The Youthbuild Case Manager’s office is located within Milwaukee Community Crossroads’s main site, located at 807 S 14th St, Milwaukee along with programming and community members. The position is expected to travel to other MCC program sites throughout Milwaukee County occasionally. Must be comfortable navigating to worksites to perform check-ins with their caseload.

Valid driver’s license and ability to drive 15-person van to take members to appointments, testings, professional development and assist with any other tasks.
