Job Title: Qualified Mental Health Professional (QMHP) – Community Stabilization Program
Job Description
Tribe Healthcare is seeking a compassionate and motivated Qualified Mental Health Professional (QMHP) to provide community based stabilization services to individuals experiencing emotional, behavioral, or mental health challenges. The QMHP works directly with individuals in their homes and community settings to provide short-term, person centered support that promotes stability, recovery, independence, and successful engagement with ongoing behavioral health services.
The QMHP partners with individuals, families, healthcare providers, and community organizations to develop individualized service plans, strengthen coping skills, improve daily functioning, and connect individuals with resources that support long-term wellness.
Essential Duties & Responsibilities
- Provide individualized, community-based behavioral health support and stabilization services.
- Complete assessments to identify strengths, needs, and barriers to recovery.
- Develop, implement, and monitor individualized service plans.
- Assist individuals in developing healthy coping skills, emotional regulation, and problem-solving techniques.
- Support individuals in achieving recovery goals and increasing independence.
- Develop and review individualized safety plans as needed.
- Coordinate care with healthcare providers, therapists, case managers, family members, and community partners.
- Connect individuals with housing, employment, education, transportation, financial assistance, healthcare, and other community resources.
- Educate individuals and families on wellness, recovery, and available community supports.
- Maintain accurate, timely, and professional documentation of all services provided.
- Participate in team meetings, case consultations, and ongoing training.
- Maintain confidentiality and professional ethical standards at all times.
- Build positive therapeutic relationships that promote trust, resilience, and recovery.
Minimum Qualifications
- Qualified Mental Health Professional (QMHP-A, QMHP-C, QMHP-T, or QMHP-E) in the Commonwealth of Virginia.
- Bachelor's degree in Human Services, Psychology, Social Work, Counseling, or a related field, or otherwise meet QMHP qualification requirements.
- Experience working with individuals with mental health or behavioral health needs is preferred.
- Valid driver's license, reliable transportation, and the ability to travel throughout the service area.
- Strong organizational, communication, and documentation skills.
Knowledge, Skills & Abilities
- Knowledge of behavioral health recovery principles and person-centered care.
- Strong crisis prevention, de-escalation, and conflict-resolution skills.
- Excellent verbal and written communication skills.
- Ability to work independently while managing a flexible schedule.
- Strong problem-solving, organizational, and time management abilities.
- Ability to collaborate effectively with individuals, families, and community partners.
- Proficiency with electronic documentation systems and basic computer applications.
Work Environment
- Services are provided primarily in community settings, including individuals' homes, schools, healthcare facilities, and other approved locations.
- Regular travel throughout the service area is required.
- Evening or weekend availability may be required based on participant needs.

Pay: $25.00 - $40.00 per hour
