North Pointe Health and Rehabilitation is currently looking for a Full - Time, Care Plan Coordinator / MDS / Restorative Registered Nurse. We are a fast paced 60 bed Skilled Nursing Facility and looking for a detailed oriented & organized applicant. Job position requires 1 weekend of On-Call every 6 weeks. Duties of position include: Initiation and maintenance of Care Plans, Admission, Quarterly, & Annual Resident Assessments, oversee's our Restorative Program and other multiple assigned task.
