Job Summary
We are seeking a dedicated and dynamic Nurse Practitioner or Physician Assistant to join our multidisciplinary healthcare team. In this vital role, you will provide comprehensive patient care across various settings, including outpatient clinics, inpatient units, and specialized care environments. Your expertise will support diagnosis, treatment, and management of diverse medical conditions, ensuring high-quality, compassionate care for patients of all ages. Responsibilities
- Conduct thorough physical examinations and patient assessments to determine health status and identify medical issues.
- Develop and implement individualized treatment plans in accordance with best practices and current clinical guidelines.
- Administer medications, including injections, and perform procedures such as suturing
- Utilize electronic health record (EHR) systems like Epic, Cerner, or eClinicalWorks to document patient encounters accurately and efficiently.
- Manage acute pain cases, perform triage assessments in urgent care or emergency settings, and coordinate follow-up care or referrals as needed.
- Provide education on health coaching, disease management, medication administration, and preventive care to patients and their families.
- Collaborate with specialists across disciplines such as gastroenterology, infectious disease, geriatrics, behavioral health, hospice & palliative medicine, and occupational health to optimize patient care plans.
Experience
- Proven clinical experience as a Nurse Practitioner or Physician Assistant in diverse healthcare settings such as hospital medicine, emergency medicine, urgent care, or outpatient clinics.
- Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth for documentation and order entry.
- Experience working with vulnerable populations including pediatric patients (pediatrics/toddler care), seniors (geriatrics/memory/dementia care), individuals with disabilities or developmental disabilities.
- Skilled in performing advanced procedures such as IV insertion/infusion therapy, suturing, phlebotomy, catheterization, spinal taps (lumbar punctures), dermal fillers or esthetic laser treatments is preferred but not mandatory.
- Knowledge of CPT coding (Current Procedural Terminology), ICD coding (International Classification of Diseases), ICD-9/10 for accurate billing documentation.
- Familiarity with telehealth platforms for remote patient consultations is advantageous.
