REQUIREMENTS AND DUTIES:
- Availability of at least two days, or ten productivity units a week, and one weekend a month for per diem registered nurses. Visits are typically conducted between the hours of 8 am-5 pm.
- Reliable transportation, current driver’s license, auto insurance
- At least one year of experience with direct patient care as a registered nurse (RN)
- Able to perform wound care, blood draw, and catheter care
- Make home visits to clients in the community, as well as Senior Living, Assisted Living, and Independent Living facilities.
- Open cases and perform thorough evaluations of patients' functional status
- Evaluate home environment for hazards or barriers to more independent living.
