Join our compassionate healthcare team as a Hospice Intake and Field Nurse (LVN), Hybrid! In this dynamic role, you will be the vital link between patients, families, and the interdisciplinary care team, ensuring seamless intake processes and delivering high-quality bedside care in diverse settings. This position offers a hybrid schedule, combining in-office intake assessments with hands-on patient visits in the field. Your expertise in acute and post-acute care, along with your strong clinical skills, will directly impact the comfort and dignity of patients during their end-of-life journey. Responsibilities
- Conduct comprehensive patient assessments during intake to determine eligibility and develop individualized care plans.
- Collaborate with healthcare providers to coordinate admissions, discharge planning, and ongoing care needs.
- Perform in-home or facility visits to evaluate patient condition, vital signs, and overall well-being, ensuring compliance with safety protocols.
- Gather detailed medical histories and review medical records to inform care strategies while maintaining HIPAA confidentiality.
- Manage case documentation accurately within electronic medical records systems, including diagnostic evaluations and nursing notes.
- Assist with utilization management by evaluating patient needs for appropriate level of care—whether inpatient, outpatient, or post-acute services.
- Educate patients and families on disease management, medication adherence, and available resources to support quality of life.
Qualifications
- Valid LVN license with current state licensure required; additional certifications in critical care or emergency medicine preferred.
- Demonstrated experience in hospital settings such as ICU, emergency room, Level I or Level II trauma centers, or medical-surgical units.
- Strong background in acute care environments including infectious disease care, behavioral health, geriatrics, PICU (Pediatric Intensive Care Unit), or OR experience is highly desirable.
- Knowledge of physiology, vital signs assessment, discharge planning, diagnostic evaluation, and case management principles.
- Familiarity with hospital medicine practices and outpatient clinic operations; post-acute and home health experience is a plus.
- Excellent patient service skills with the ability to perform thorough patient assessments and manage complex cases effectively.
- Proficiency in medical terminology, medical records management, HIPAA compliance, and utilization management processes.
