What You'll Be DoingDeliver Exceptional Rehabilitation Care
- Perform comprehensive evaluations and mobility assessments
- Develop individualized treatment plans focused on safety and functional progress
- Provide hands-on therapy, gait training, and therapeutic exercise progression
- Recommend adaptive equipment and home modifications
- Educate patients and caregivers to maximize outcomes
Collaborate Across The Continuum
- Work closely with nurses, physicians, caregivers, and interdisciplinary clinicians
- Monitor progress and modify treatment plans as needed
- Document visits accurately and efficiently
- Help patients regain confidence, strength, and independence
Required Qualifications
- Active Minnesota Physical Therapist license
- Valid driver's license and reliable transportation
- Strong communication and clinical reasoning skills
- Ability to work independently and manage your schedule effectively
Preferred Qualifications
- Home health experience
- Outpatient, inpatient, SNF, or rehabilitation experience
- Passion for relationship-driven, patient-centered care
