Licensed Practical Nurse (LPN)
Location: Clinton Township, MI
Overview:
Our well-established home health agency is seeking a compassionate Licensed Practical Nurse to provide private duty care for a patient in their home. Our role offers a slower-paced, patient-focused environment with predictable weekend daytime hours. You will deliver one-on-one nursing care while supporting comfort, safety, and independence.
Position Offers:

Schedule: Every other weekend, Saturday and Sunday, 7:00 AM–7:00 PM

One-on-one private duty care setting
Key Responsibilities:

Provide direct, hands-on nursing care to a private duty patient in their home

Monitor vital signs regularly and report changes to the supervising RN or provider
-
Administer medications safely according to the plan of care

Maintain catheter care and other clinical tasks per the plan of care
-
Document care, observations, and interventions in the EHR
-
Educate the patient and family on care needs and treatment plans
-
Assist with activities of daily living, meal preparation, and light housekeeping
-
Provide transportation and companionship to appointments and activities
Qualifications:
-
Active, unrestricted LPN license in Michigan
-
Minimum 2 years of LPN experience

Home health or private duty experience preferred
-
Valid driver's license with a clean driving record

Current BLS/CPR certification
-
Comfortable working in a home environment with household pets
