Job Summary
Responsible for assisting patients in reestablishing services by assessing their needs, coordinating documentation and telehealth appointments, managing pending cases, and ensuring effective communication with healthcare providers for comprehensive care.
Responsibilities
Patient Reengagement:
- Assist patients in reestablishing services by assessing their current skilled needs.
- Communicate with patients to understand their healthcare requirements and preferences.
Documentation and Coordination:
- Obtain necessary paperwork from primary care physicians.
- Schedule telehealth appointments as needed to facilitate care.
Pending Case Management:
- Manage pending cases within the GHC and EHC buckets.
- MK progress notes for patients post 30 days.
- Follow up on patients who have refused services.
- Address cases awaiting family decisions pending over 30 days.
Effective Communication:
- Collaborate with team members and healthcare providers to ensure timely follow up and comprehensive
patient care.
- Maintain accurate and organized documentation of all patient interactions and statuses.
All team members are required to assist in covering absent employees and assist with training new staff within
their area of expertise.
Qualifications
Prior sales experience or prior homecare experience is preferred

- Ability to navigate multiple computer screens while using the phone
- Typing minimum of 40 WPM
- Microsoft Office (basic)
Education
- High school graduate or equivalent; two years of college preferred.
Pay/Salary
$21- $25 per hour
