Heal At Home is growing, and we're looking for compassionate Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our home health team.

RN Range - $36-40

Role and Responsibilities:
- Deliver nursing care in accordance with the plan of care and physician’s orders.
- Monitor patient health, vital signs, and report changes to the RN or physician.
- Administer medications, treatments, and wound care.
- Educate patients and families on disease management and self-care techniques pertinent to diagnosis and the care plan.
- Assist with daily living activities such as bathing, dressing, and mobility.
- Document patient care accurately and timely using electronic health records.
- Collaborate with the healthcare team to coordinate comprehensive care and appropriately report identified needs for other disciplines to the registered nurse.
- Participate in care planning and discharge coordination.
- Use equipment and supplies effectively and efficiently.
- Maintain appropriate and applicable infection control standards including hand hygiene.
- Maintain a safe and clean working environment in compliance with healthcare procedures and regulations.
- Maintain confidentiality of client information in compliance with HIPAA and company policies.
- Participate in professional growth, development, trainings, education and meetings as requested.
- Perform other duties as assigned by DON, RNCM or Administrator.
