$10,000 Sign-On Bonus!
We are a growing house calls medical practice seeking an experienced and compassionate Nurse Practitioner or Physician Assistant to provide in-home primary care to a medically complex, homebound patient population. Bonus paid over a 24-month employment agreement.
This position serves patients within the League City to Galveston area. Applicants must live locally to ensure timely, efficient travel within the service area.
Responsibilities:
- Provide comprehensive primary care services in patients’ homes
- Manage chronic conditions and acute issues for a geriatric population
- Perform assessments, diagnose, and develop individualized care plans
- Prescribe medications and order diagnostics as appropriate
- Document visits accurately and timely in the EHR
- Coordinate care with interdisciplinary team members, home health, and specialists
- Participate in quality initiatives and value-based care programs
- Maintain compliance with CMS, payer, and organizational guidelines
Qualifications:
- Current Texas Nurse Practitioner or Physician Assistant license in good standing
- National certification (AANP, ANCC, or NCCPA)
- Experience in Primary care
- Strong clinical assessment and time-management skills
- Comfortable working independently in the field
- Reliable transportation and valid Texas driver’s license
- Must reside within or near the Texas City service area
Preferred Experience:
- House calls, home-based primary care, or geriatrics
- Medicare and value-based care models
- Experience with medically complex or homebound patients
