Responsibilities:
- Design and implement individualized rehabilitation plans to meet the unique needs of each patient
- Conduct thorough evaluations to identify patient needs and establish goals
- Provide therapy services, including but not limited to occupational, physical, and speech therapy
- Collaborate with multidisciplinary teams to ensure comprehensive care
- Develop and maintain patient records, including progress notes and treatment plans
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
Requirements:
- Experience as a Rehabilitation or Physical Therapist or related field
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize tasks effectively
- $58 - $60 HOURLY
