Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
- Monitor and document vital signs, medication administration, and medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems.
- Provide specialized care for clients with dementia, Alzheimer’s disease, or other cognitive impairments, including memory care techniques and behavioral management strategies.
- Support patients during end-of-life care with compassion, dignity, and adherence to hospice care protocols.
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment aligned with home health regulations.
- Maintain patient confidentiality and adhere to HIPAA guidelines while managing sensitive medical documentation.
- Communicate effectively with clients and their families to ensure personalized care plans are followed and needs are met promptly.
Skills
- Proven experience working with geriatric patients, individuals with disabilities, or in assisted living environments.
- Strong knowledge of medical terminology, catheter care, CPR, first aid, and basic life support techniques.
- Familiarity with home health agency operations, hospice care practices, and home & community care standards.
- Ability to perform patient transfers safely using various methods such as Hoyer lifts or other assistive devices.
- Excellent patient observation skills to monitor health status changes or behavioral cues.
- Experience working with EMR/EHR systems for documentation and medical records management.
- Compassionate caregiver with excellent communication skills capable of building trusting relationships with clients and their families.
