The Health Records Clerk is responsible for organizing and managing health information data, including ensuring the integrity of the legal health record, and assisting in providing comprehensive information to the care team. Assists patients and other agencies with Medical, Dental and Behavioral health record related needs.
Accountabilities Include:

Sorts, files, and collates a variety of health records and information into patient health record in appropriate sequence to ensure completeness and in accordance with established procedure.

Filing paperwork and reports from other agencies into patient health record accurately within the established time frame.

Creating digital copies of paperwork and storing the records electronically per policy.

Ensuring that all health records are protected and kept confidential.

Process incoming and outgoing correspondence to patients and other agencies including, but not limited to; fax, mail, inter office courier and my-chart messages.

Ensures all required information is included in the legal health record noting deficiencies and referring to appropriate area for follow-up.

Processes both incoming and outgoing release request for health records including but not limited to court orders, subpoenas, patients request and other agencies to the extent allow by HIPAA standards.

Provide excellent customer service to external partners and internal team member by prompt response to telephone calls Answers telephones, email request and messages sent through Electronic Health Record system.

Ensures that all incoming or outgoing requests to release health records are logged in the Release of Information [ROI] Module tracking system.

Retrieves and copy l health information from the patient l health records to answer authorized requests in support of patient care activities and patient account settlements.

Completes assigned trainings via La Clinica’s learning module system (Relias) by required due date.

Serves as content expert and provides internal training regarding processes related to health records management to comply with the organization standards.

Other duties as assigned.
Qualifications:
Must be able to interact with a variety of individuals at various levels under stressful circumstances while exercising sound judgement, tact, and diplomacy; work productively both independently and in a team setting; communicate with clarity, both verbally and in writing to groups and individuals. Strong data entry skills and attention to details.
Education and/or Experience:
Required:

Minimum: High School Diploma.
-
Excellent interpersonal communication and problem-solving skills.
-
Skills to intervene and promote reconciliation, compromise and positive outcomes in difficult interactions.
Preferred:
-
Six months to one-year experience in a medical office setting or related work experience.
-
Basic knowledge of adult learning models.
