Position Summary
St. Luke’s Health System in Boise, Idaho is seeking a Full-time Meds-to-Beds Pharmacist to join our Outpatient pharmacy. This position plays a key role in delivering high-quality pharmaceutical care across multiple outpatient service areas, including medication lockers, traditional outpatient pharmacy services.
The ideal candidate will be comfortable with the clinical aspects of pharmacy practice, thrive in a collaborative healthcare environment, and be motivated to support innovative outpatient pharmacy services.

Key Responsibilities
The Outpatient Pharmacist is responsible for the delivery of safe, effective, and patient-centered pharmaceutical care, including but not limited to:

Review and evaluate the appropriateness of prescribed medication therapies

Interpret prescriptions and accurately compound and dispense medications

Provide medication information, counseling, and consultation to patients, nurses, and medical staff

Perform cognitive clinical pharmacy services and document interventions according to medical center standards

Administer medications and vaccinations as appropriate

Supervise, direct, and authorize the work of pharmacy technicians, interns, externs, and support staff

Maintain accurate records related to medication therapy, prescriptions, patient charges, inventory, and medical records

Support Meds-to-Beds and Transitions of Care services to improve patient outcomes and continuity of care

Assist with training and education of technicians, interns, externs, residents, and other pharmacy staff

Serve as a role model and mentor to students, technicians, residents, and new practitioners
Qualifications
Education
-
Degree from an accredited School or College of Pharmacy (PharmD or equivalent)
Experience
-

Experience with EPIC electronic medical record system is preferred
Licenses & Certifications

Valid Pharmacist License in the state of practice (Idaho or eligible for licensure)

Basic Life Support (BLS)certification required, through one of the following:

American Safety and Health Institute (ASHI)

American Heart Association (AHA)

American Red Cross
