We are seeking a compassionate and dedicated Social Services professional to join our healthcare team. This position plays an important role in supporting residents and their families by addressing individual psychosocial needs, coordinating services, assisting with discharge planning, and promoting overall resident well-being.
The ideal candidate will have experience working in a skilled nursing facility, strong assessment and communication skills, and a genuine interest in helping residents navigate their healthcare and personal needs.
Build positive relationships between residents and staff to support each resident's individual needs.
Identify options and resources that best meet the physical, emotional, and psychosocial needs of each resident.
Maintain communication with residents' families regarding changes in health, current goals, and care planning.
Make appropriate referrals and coordinate services with outside agencies and community resources.
Provide or arrange counseling and supportive services as needed.
Assist residents with financial and legal matters and connect them with appropriate resources.
Participate as a member of the interdisciplinary team to support comprehensive resident care.
Review facility policies and procedures to help ensure compliance with applicable state and federal regulations.
Support residents and families throughout the care planning and discharge planning process.
Advocate for residents' needs, preferences, rights, and overall quality of life.
Bachelor's degree required; Master's degree strongly preferred.
LSW licensure required; LCSW or LICSW strongly preferred.
Minimum of two years of experience working in a skilled nursing facility.
Experience with short-term rehabilitation and discharge planning required.
Excellent communication, documentation, and assessment skills.
Strong interpersonal and organizational skills.
Genuine concern for and interest in working with people.
Ability to work effectively with residents, families, healthcare professionals, and community service providers.
