Our Swing Bed RN plays a key role in helping patients transition from acute hospital care toward recovery, rehabilitation, and a successful return home. This is more than bedside nursing—you'll be the connection between patients, families, therapists, physicians, and community resources.
What You'll Be Doing
- Provide compassionate, individualized nursing care for Swing Bed patients.
- Coordinate care with physicians, therapists, hospitalists, case management, and other healthcare professionals.
- Review referrals and help coordinate timely Swing Bed admissions.
- Coordinate specialized nursing care, including wound care, medication management, and ongoing monitoring.
- Support patients through physical, occupational, and speech therapy.
- Educate patients and families about treatment, recovery, and discharge plans.
- Coordinate discharge needs, including home health, DME, appointments, LTC referrals, and follow-up calls.
- Complete required MDS and clinical documentation accurately and on time.
- Lead weekly interdisciplinary team meetings and help keep everyone on the same page.
- Participate in outreach efforts to build relationships with referral facilities and grow our Swing Bed program.
- Help ensure every patient receives coordinated, comprehensive care throughout their recovery.
What We're Looking For
Required:
- Graduate of an accredited Nursing School
- Current Georgia RN License
- Current BLS Certification
- Current ACLS Certification
Preferred:
- Previous Registered Nurse experience
