Job Title: Licensed Practical Nurse (LPN) – Skilled Nursing Facility / Long-Term Care
Location: Ballard Nursing Center (under new ownership)
Shift: 11:00 PM – 7:00 AM (Night Shift)
Compensation: $30.00 per hour
Position Summary
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Skilled Nursing Facility (SNF) and Long-Term Care (LTC) team on the night shift. The LPN is responsible for providing quality nursing care to residents, administering medications and treatments, monitoring resident conditions, and supporting the overall health and well-being of residents in accordance with facility policies and applicable regulations.
Essential Duties and Responsibilities
- Administer prescribed medications and treatments accurately and timely.
- Monitor residents' health status and report changes in condition to the Registered Nurse (RN) or physician.
- Perform nursing assessments and document resident care in medical records.
- Assist in developing and implementing individualized care plans.
- Provide wound care and other nursing procedures as assigned.
- Supervise and support Certified Nursing Assistants (CNAs) during assigned shifts.
- Respond promptly to resident needs, emergencies, and call lights.
- Maintain accurate and complete documentation in accordance with facility policies and state regulations.
- Ensure infection control, safety, and quality care standards are maintained.
- Communicate effectively with residents, families, physicians, and interdisciplinary team members.
Qualifications
- Current and active Licensed Practical Nurse (LPN) license in the state of employment.
- Graduate of an accredited Practical Nursing program.
- Previous experience in Skilled Nursing, Long-Term Care, Rehabilitation, or Geriatric Nursing preferred.
- Current CPR/BLS certification preferred.
- Strong clinical assessment, communication, and organizational skills.
- Ability to work independently and collaboratively during overnight hours.
Physical Requirements
- Ability to stand, walk, bend, lift, and assist residents throughout the shift.
- Ability to lift and move up to 50 pounds with or without reasonable accommodation.
- Ability to perform duties in a fast-paced healthcare environment.
