Position Overview:
The Family Support Program Case Manager Liaison plays a vital leadership role in ensuring the successful delivery of U.N.I.'s Family Support Programs by providing comprehensive case management, fostering meaningful family engagement, and overseeing program operations. This position serves as the primary point of contact for families, Regional Centers, community partners, and internal staff while ensuring services are person-centered, compliant, and aligned with U.N.I.'s mission and values.
The Family Support Program Case Manager Liaison is responsible for coordinating services, monitoring client progress, supervising Family Support Specialists, maintaining quality assurance standards, and supporting the development and implementation of program initiatives. This role also leads community outreach efforts, organizes resource fairs and special events, and builds collaborative relationships with schools, Regional Centers, and other community organizations to enhance access to resources and services.
Essential Duties & Responsibilities:
Case Management & Family Engagement
§ Support clients and their families across all U.N.I. Family Support Program offerings
§ Perform case management duties across all U.N.I. Family Support Program offerings
§ Monitor service quality by tracking client progress and adherence to established care plans
Program Leadership & Team Supervision:
§ Demonstrate strong organizational, leadership, and administrative skills
§ Maintain a proven track record of managing teams and delivering high-quality client services
§ Supervise and manage a team of Family Support Specialists across various U.N.I. program offerings
§ Provide guidance and training to staff on best practices in client care and accurate documentation
§ Create necessary program guides and training materials on an as-needed basis
Service Coordination & Quality Assurance:
§ Oversee scheduling, service coordination, and documentation processes to maintain operational efficiency
§ Ensure the accuracy of all program documentation through regular auditing procedures
§ Facilitate and monitor the progress of selected Family Support Programs to ensure their quality and effectiveness
§ Maintain accurate records of all client/community stakeholder interactions
Community Stakeholders & U.N.I. Team Collaboration:
§ Serve as the Primary Point of Contact / Lead for all U.N.I. Family Support Program offerings
§ Report to the relevant Program Lead(s) and participate in bi-weekly or as needed meetings
§ Attend internal and external meetings, including but not limited to clients and their families, Regional Center staff, and other relevant community-based program partnerships
§ Build and maintain strong working relationships with partnering organizations, including but not limited, to local schools, school districts, and Regional Center
Event & Resource Fair Coordination:
§ Ensure all Family Support Program offerings have a community-based resource fair planned for each quarter (4/year)
§ Plan and coordinate special U.N.I. hosted events to support various program goals
§ Maintain active vendor directory for regular U.N.I. use across all event hosting opportunities
§ Build and maintain strong working relationships with outside vendors to ensure quality of events
§ Collaborate with Regional Center to find partnering opportunities for community outreach efforts
Other Responsibilities:
§ Exhibit strong clerical skills with proficiency in Google Workspace (Docs, Sheets, Drive, etc.)
§ Exhibit a willingness to be trained and adapt to program needs
§ Conduct bi-weekly vehicle inspections, if applicable
§ Perform other duties as assigned

Position Requirements:
§ Bachelor’s degree or higher in Psychology, Social Work, Human Services, or a related field (required)
§ Knowledge of Regional Center systems, Title 17 regulations, and DDS CFS Guidelines (required)
§ Bi-lingual language proficiency (Spanish highly preferred)
Licenses & Certifications:
§ Valid California Driver’s License, reliable transportation, and proof of insurance (required)
§ DOJ / FBI fingerprint clearance required prior to hire
§ Current CPR/First Aid Certification (or willingness to obtain)
§ Completion of U.N.I. paid mandatory training (Title 17, HIPAA, CLAS, SIR, etc.) within 90 days of hire
Working Conditions:
§ Primarily office-based role. Frequent travel to family homes and community sites required
§ May include occasional evenings or weekends dependent on client or company needs
§ May involve moderate physical activity and variable work environments and weather conditions
§ Schedule(s) / Hour(s) are subject to change based on client needs/availability
§ Work location, work schedule, and hours are subject to change based on client/company needs
Pay: $25.00 - $33.00 per hour
