Overview
We are seeking a dedicated Service Coordinator to join our team. The Service Coordinator will play a crucial role in managing and coordinating services for individuals with physical and/or cognitive impairments. This position offers an opportunity to make a meaningful impact on the lives of others.
Participant Support & Service Planning
- Conduct initial, annual, and ongoing assessments to identify participant strengths, needs, risks, and goals.
- Develop comprehensive Service Plans in accordance with NYSDOH NHTD Waiver policies.
- Facilitate person‑centered planning meetings with participants, families, caregivers, and providers.
- Ensure all services are clinically appropriate, cost‑effective, and aligned with waiver requirements.
Care Coordination
- Coordinate all waiver and non‑waiver services, including medical, behavioral health, social supports, housing, transportation, and community resources.
- Maintain regular contact with participants through home visits, phone calls, and collateral communication.
- Monitor service delivery to ensure providers meet quality, timeliness, and documentation standards.
- Respond promptly to changes in participant needs, incidents, or emerging risks.
Documentation & Compliance
- Complete all required assessments, Service Plans, Addendums, and progress notes within mandated timeframes.
- Maintain accurate, organized, and audit‑ready participant records.
- Ensure compliance with NYSDOH regulations, HIPAA, and agency policies.
- Submit required reports, incident documentation, and follow‑up actions as needed.
Advocacy & Resource Navigation
- Advocate for participant rights, preferences, and access to services.
- Assist participants with navigating Medicaid, housing programs, benefits, and community supports.
- Provide education to participants and families regarding waiver services and responsibilities.
Collaboration
- Work closely with waiver providers, Managed Care Organizations, hospitals, nursing facilities, and community agencies.
- Participate in interdisciplinary team meetings and case conferences.
- Maintain professional communication and positive working relationships with all stakeholders.
Qualifications
- Bachelor’s degree in Human Services, Social Work, Psychology, Gerontology, or related field required.
- Minimum one year of experience working with individuals with disabilities, seniors, or community‑based services.
- Knowledge of Medicaid programs, long‑term care systems, and NYS waiver services preferred.
- Strong organizational, communication, and documentation skills.
- Ability to conduct home visits and travel within the community.

Work Environment

Pay: $25.27 - $30.43 per hour
