Under the direction of the Supervisor of Community Health & Wellness, the Community Health Worker (CHW) serves as a trusted link between the community and health/social service systems, helping improve access to care, reduce barriers, and promote whole-person wellness.
- Support individuals through in-office, in-home, and community-based outreach

Connect consumers to community resources, benefits, and support services

Conduct Social Determinants of Health (SDOH) screenings and follow-up support

Assist with appointment scheduling, reminder calls, and wellness program participation

Advocate for individuals and help improve access to behavioral health, substance use, and physical health services

Provide encouragement, education, and recovery-focused support

Facilitate communication between consumers and healthcare/social service providers

Assist with wellness groups, health monitoring, and care coordination activities
Required Education/Certification/Licensure
- Michigan Community Health Worker certification or ability to be certified within 3 months of hire.
- High school diploma or GED.
- CPI Training, upon hire.
- Must possess a valid U.S motor vehicle operator’s license, and successfully complete motor vehicle record check.
Preferred Education / Certification/Licensure
- Associates degree.
- Michigan Community Health Worker Certification.
- Bilingual (Spanish/English).
