Job Overview
We are seeking a dedicated and experienced Registered Nurse (RN) to serve as a Case Manager for a four-week assignment in <LOCATION>. This role involves coordinating comprehensive patient care, facilitating effective discharge planning, and ensuring optimal health outcomes through meticulous assessment and management. The ideal candidate will possess strong clinical expertise across various specialties, including critical care, trauma, emergency medicine, and post-acute care. Duties
- Conduct thorough patient assessments to determine individual needs, utilizing knowledge of physiology, medical terminology, and clinical protocols.
- Develop and implement personalized care plans that align with hospital standards and patient goals.
- Coordinate multidisciplinary care efforts across inpatient and outpatient settings, including ICU, PICU, Geriatrics, Behavioral Health, and Medical-Surgical units.
- Facilitate discharge planning by evaluating post-acute care options, managing referrals, and ensuring seamless transitions for patients.
- Monitor vital signs, review diagnostic evaluations, and oversee patient progress to adjust care strategies accordingly.
- Collaborate with healthcare teams to optimize utilization management and ensure compliance with HIPAA regulations.
- Maintain accurate medical records, document patient interactions thoroughly, and support quality assurance initiatives.
- Engage in patient service activities that promote safety, comfort, and satisfaction throughout the care continuum.
- Participate in intake processes for new admissions and conduct ongoing patient assessments to inform case management decisions.
- Support hospital initiatives related to infectious disease care, trauma protocols, or specialty areas such as cardiology or pediatrics as needed.
Skills
- Proven critical care experience with a strong foundation in ICU procedures and Level II or Level I trauma center protocols.
- Extensive hospital experience across various units including emergency room, medical-surgical wards, PICU, geriatrics, behavioral health, or outpatient clinics.
- Demonstrated knowledge of medical-surgical nursing practices, diagnostic evaluation procedures, and acute care management.
- Familiarity with managed care processes, utilization management strategies, and discharge planning workflows.
- Excellent patient assessment skills combined with a solid understanding of physiology and medical terminology.
- Strong organizational abilities with attention to detail in documentation and medical records management.
- Effective communication skills for collaborating with multidisciplinary teams and engaging patients compassionately.
- Knowledge of HIPAA compliance standards to safeguard patient information at all times.
- Ability to adapt quickly within fast-paced environments while maintaining high-quality patient care standards.
