About the Role
We are seeking a compassionate and experienced Registered Nurse (RN) Clinical Care Manager to provide care coordination and clinical support for One Care members with complex medical, behavioral health, and social needs. This role works closely with clients, healthcare providers, interdisciplinary teams, and community resources to develop and coordinate individualized care plans that promote health, independence, and quality of life.
The Clinical Care Manager serves as a key advocate for members, helping to coordinate services, improve health outcomes, and ensure seamless transitions across care settings.
Key Responsibilities
- Engage and support One Care members through outreach, assessment, and ongoing care coordination.
- Complete comprehensive health assessments addressing medical, behavioral health, and social needs.
- Develop, implement, and monitor individualized care plans in collaboration with clients and care teams.
- Coordinate interdisciplinary team meetings and facilitate communication among providers, clients, families, and support staff.
- Monitor health status, medication needs, treatment goals, and changes in level of care.
- Collaborate with primary care providers, specialists, hospitals, rehabilitation facilities, and community resources.
- Support safe transitions following hospitalizations or other changes in care.
- Provide health education, self-management coaching, and support to clients and families.
- Conduct medication reviews and reconciliation.
- Promote person-centered, trauma-informed, and recovery-oriented care practices.
- Supervise assigned support staff and serve as a clinical resource to team members.
Qualifications
- Current Registered Nurse (RN) license in Massachusettsrequired.
- Bachelor of Science in Nursing (BSN) required; MSN preferred.
- Minimum of 5 years of clinical nursing and/or case management experience preferred.
- Experience developing care plans and coordinating services for individuals with complex medical and behavioral health needs preferred.
- Strong clinical assessment, communication, organization, and collaboration skills.
- Ability to work independently and as part of a multidisciplinary team.
- Knowledge of care coordination, behavioral health, community resources, and person-centered practices preferred.
- Bilingual/bicultural candidates and individuals with lived experience related to mental health conditions are encouraged to apply.
Additional Requirements
- Valid driver’s license for 6+ months and reliable transportation required.
- CPR certification required within two weeks of hire.
- Must be able to meet physical requirements of the position, including community-based client support and mobility demands.
