Responsibilities
- Deliver compassionate patient care by assisting with ADLs such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts.
- Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates.
- Observe and report changes in patient condition promptly to the healthcare team, ensuring timely interventions.
- Administer medications following prescribed protocols while maintaining strict adherence to HIPAA regulations for patient privacy.
- Assist with specialized care needs such as catheter care, hospice support, dementia and Alzheimer’s care, memory care, and working with individuals with developmental disabilities or disabilities.
- Document medical records and care activities using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems efficiently and accurately.
- Support patients’ nutritional needs through meal preparation and light cleaning duties to maintain a safe and hygienic environment.
- Operate medical equipment safely, including Hoyer lifts and other mobility aids, ensuring patient safety at all times.
- Maintain compliance with state licensing requirements and certifications such as CPR and medical terminology knowledge.
- Collaborate effectively with team members to ensure seamless continuity of care in home health or long-term care settings.
Experience
- Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred.
- Proven ability to work with individuals with disabilities or chronic conditions such as dementia or Alzheimer’s disease.
- Familiarity with medical documentation standards and EHR/EMR systems for accurate record keeping.
- Experience providing hospice or memory care services is a plus.
- Knowledge of medical terminology, medication administration procedures, and vital signs monitoring is essential.
- Demonstrated ability to perform patient observation diligently while maintaining compassion and professionalism.
