Overview
Sheraton Caregivers is seeking an experienced, compassionate, and relationship-driven Clinical Concierge Care Manager (RN/LPN) to join our leadership team.
This field-based leadership position is responsible for delivering exceptional client care through clinical oversight, caregiver supervision, care coordination, and relationship management. The Clinical Concierge Care Manager serves as the primary clinical resource for assigned clients and caregivers, ensuring every client receives safe, personalized, and concierge-level care in their home or senior living community.
Working closely with clients, families, physicians, hospitals, rehabilitation centers, assisted living communities, memory care communities, hospice providers, and our internal leadership team, you will help ensure continuity of care, outstanding client experiences, and exceptional caregiver performance.

Responsibilities
- Conduct comprehensive client assessments and reassessments.
- Develop, implement, and update individualized care plans.
- Complete supervisory visits in clients' homes and senior living communities.
- Supervise, mentor, coach, and support caregivers to promote quality care and professional growth.
- Introduce caregivers to new clients and ensure successful care transitions.
- Build trusted relationships with clients, families, physicians, and community partners.
- Coordinate care with hospitals, rehabilitation centers, hospice providers, assisted living communities, and other healthcare professionals.
- Monitor client satisfaction and proactively resolve concerns to ensure exceptional service.
- Ensure caregivers follow care plans, company policies, and best practices.
- Maintain accurate clinical documentation using electronic care management software.
- Collaborate with Scheduling, Operations, and Field Leadership to maintain continuity of care.
- Participate in quality improvement initiatives and help maintain Sheraton Caregivers' high standards of service.
- Serve as a professional ambassador for Sheraton Caregivers throughout the community.
QualificationsRequired
- Current Connecticut Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing.
- Minimum three (3) years of nursing experience.
- Minimum two (2) years of experience supervising caregivers, CNAs, HHAs, or direct care staff in a home care, hospice, assisted living, rehabilitation, or senior care setting.
- Experience conducting client assessments and developing individualized care plans.
- Strong leadership, coaching, and caregiver mentoring skills.
- Excellent communication, interpersonal, and organizational skills.
- Strong assessment, critical thinking, and clinical decision-making abilities.
- Ability to work independently while managing multiple priorities.
- Valid driver's license, reliable transportation, and willingness to travel throughout Fairfield County.
- Proficiency with electronic documentation systems and Microsoft Office.
Preferred Qualifications
- Private duty home care experience.
- Certified Dementia Practitioner (CDP) credential or equivalent dementia care training is a plus.
- Experience caring for individuals living with Alzheimer's disease and other forms of dementia.
- Experience in client care coordination or case management.
- Strong relationship-building skills with clients, families, referral partners, and healthcare professionals.
Schedule
- Full-Time (40 hours per week)
- Primarily Monday–Friday, 7:00 AM – 4:00 PM
- Every other weekend rotation required
- Occasional 1:00 PM – 10:00 PM shifts as part of the shared leadership rotation
- Travel throughout Fairfield County required

Pay: $70,000.00 - $78,000.00 per year
