What You'll Be DoingRestore Mobility And Confidence
- Complete physical therapy evaluations and treatment visits in patient homes
- Develop individualized treatment plans focused on mobility, strength, balance, endurance, and fall prevention
- Help patients recover from hospitalization, surgery, injury, and chronic conditions
- Recommend assistive devices, home modifications, and safety strategies
- Educate patients and caregivers to maximize long-term success
Collaborate With An Exceptional Team
- Partner with nurses, Occupational Therapists, Speech Language Pathologists, physicians, and case managers
- Support continuity of care and positive patient outcomes
- Complete timely documentation and care planning activities
- Serve as a trusted resource for patients and families
Required Qualifications
- Active Minnesota Physical Therapist license
- Strong clinical assessment and treatment planning skills
- Ability to work independently and manage a visit-based schedule
- Reliable transportation, valid driver's license, and current auto insurance
Preferred Qualifications
- Home health experience
- Experience with geriatric and medically complex patients
- Passion for relationship-based, patient-centered care
