Job Overview
We are seeking a dedicated and compassionate Physical Therapist to join our healthcare team on a PRN basis. In this role, you will deliver high-quality rehabilitative care to seniors in a community for independent, assisted and memory care residents. Your expertise will help patients regain mobility, reduce pain, and improve their overall quality of life through personalized treatment plans. Duties
- Assess patients’ physical conditions through comprehensive evaluations, including anatomy and physiology knowledge, to develop effective care plans.
- Implement treatment plans utilizing techniques such as therapeutic exercises and mobility training.
- Collaborate with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate patient care.
- Document patient progress accurately in electronic health records system point of service documentation.
- Educate patients on treatment goals, home exercise programs.
- Manage planning processes by assessing patient needs for continued therapy.
- Skills
- Strong understanding of medical terminology and clinical documentation standards essential for accurate record-keeping per Medicare guidelines
- Proven experience in outpatient environments with a focus on orthopedics, neurology and, geriatrics.
- Ability to develop individualized treatment plans that incorporate techniques tailored to patient needs.
- Excellent client communication skills to motivate and educate patients throughout their treatment journey.
- Experience working with geriatric patients highly desirable.
- Familiarity with inpatient care procedures and acute care experience enhances your ability to adapt quickly across different clinical scenarios.
