A respected and growing long-term care organization is seeking an experienced Regional Social Worker to provide leadership, guidance, and support to facility-based social service teams across multiple skilled nursing centers. The ideal candidate is a compassionate professional with a strong understanding of regulatory requirements, psychosocial care, and interdisciplinary collaboration in the long-term care environment.
Responsibilities:
Provide regional oversight, mentorship, and training to facility social workers and department leaders.
Ensure compliance with federal, state, and local regulations governing social services in skilled nursing facilities.
Support facilities in resident assessments, care planning, and discharge processes.
Collaborate with clinical, admissions, and administrative teams to promote quality outcomes and smooth transitions of care.
Conduct on-site visits to monitor department performance and assist with survey readiness.
Serve as a resource for complex psychosocial cases and family concerns.
Contribute to the development of company-wide policies and best practices for social services.
Qualifications:
Bachelor’s degree in Social Work (BSW) required; Master’s degree (MSW) preferred.
Current Social Work licensure as required by PA and NJ
Minimum of 5 years of experience in long-term care, with at least 2 years in a leadership or multi-site role.
Strong knowledge of discharge planning, resident rights, care coordination, and documentation standards.
Excellent communication, organization, and leadership skills.
Ability to travel regionally between assigned facilities.
