General Function
Zach’s Place Case Manager is responsible for coordinating and providing services and crisis intervention to program participants.
Equipment and Job Location
This position requires the use of the telephone, copy machine, fax machine, cell phone, computer skills (word processing/spread sheets) and transportation to speaking engagements, school presentations and community events.
Primary Responsibilities
- Responding to homeless service requests made by city staff, law enforcement, businesses, schools and concerned residents in a timely manner
- Screen and conduct intakes and assessment interviews with homeless individuals, including medical/mental health history to identify social service and housing needs and make appropriate referrals
- Assist participants in their development of short and long-term case management goals that will include strategies to overcome barriers to self-sufficiency and to obtain permanent housing
- Serve as an advocate for participants in securing social services, mainstream benefits, housing opportunities, and provide support to participants in securing and/or following through on services
- Perform initial comprehensive case management assessment and develop a plan of intervention that addresses need for housing, services, crisis intervention, and continuity of care planning and coordination
- Attend and participate in housing navigation meetings through the Coordinated Entry System
- Provides field based/ mobile case management services in the areas of,
- Provide services focused on enhancing the clients’ ability to independently solve problems, utilize effective coping skills, and manage and self-coordinate own care
- Utilize evidence-based practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention and Housing First practice
- Assist clients with housing applications, complete supportive and subsidized housing paperwork, survey rental market for affordable housing, and advocate for clients with prospective landlords
- Transport participants, as needed
- Participate in program-related coalitions, meetings, partnerships, community activities and projects
- Maintain documentation and records of participant contacts and assist in the preparation of reports on participant progress and status
- Maintain documentation standards as set forth by the program and DCA - HMIS policies, including maintaining compliance with confidentiality policies and practices
- Complete progress notes on every face to face/ telephone contact with client or collateral contact by the next business day.
- Complete intake, assessment, service plans in coordination with client and within the documentation standards set forth by DCA -HMIS.
- Plan and coordinate policies and programs for Aftercare Clients
- Provide direct services to Aftercare Clients
- Coordinate services with other community agencies/programs
- Represent the Shelter program locally, regionally and nationally
- Participate in the local governance board plan development
- Provide 24-hour on-call services
- Conducting school presentations
- Conduct presentations for community organizations
- Develop and maintain community relations with key school personnel, business contacts and community organizations

Qualifications
- Minimum of a bachelors degree in, communication or social services, etc. Prefer two years experience related to service coordination, training or education
Hours and Pay
- Monday – Friday 9:00 AM- 5:00 PM
- On call (Schedule is subject to change based on availability and case)
- $48, 000 Yearly
