Responsibilities
- Conduct thorough evaluations of patients’ physical conditions, including mobility, strength, flexibility, and functional abilities.
- Develop and implement individualized treatment plans aligned with clinical standards and patient goals.
- Deliver evidence-based physiotherapy interventions such as manual therapy, trigger point therapy, therapeutic exercises, and modalities tailored to each patient’s needs.
- Monitor patient progress regularly, adjusting care plans to optimize outcomes and ensure safe recovery.
- Collaborate with multidisciplinary teams to coordinate discharge planning, follow-up care, and community reintegration strategies.
- Maintain accurate clinical documentation adhering to healthcare standards and electronic health records (EHR) management protocols.
- Educate patients and their families on injury prevention, home exercise programs, and self-care techniques to promote long-term wellness.
Requirements
- Valid license or certification to practice as a Physical Therapist in the relevant jurisdiction.
- Proven experience working in outpatient clinics or outpatient rehabilitation centers with a focus on orthopedics, neurology, or geriatrics.
- Strong knowledge of anatomy, physiology, medical terminology, and clinical documentation standards.
- Demonstrated ability to work effectively with geriatric patients and those requiring home & community care services.
- Experience with inpatient hospital settings or acute care environments is highly desirable.
- Familiarity with EMR systems and electronic health records (EHR) management for accurate documentation.
- Excellent patient care skills with a compassionate approach to diverse populations including pediatric and geriatric patients.
- Ability to perform discharge planning efficiently while coordinating with other healthcare providers for seamless patient transitions.

Pay: $45.00 - $47.00 per hour
