Family and Children's Services, Inc. (FACS) is seeking a dedicated and compassionate Case Manager for our Wellness and Recovery Support Services Program. This program will take a non-clinical, multi-pronged, holistic approach to help individuals and families recover from substance and alcohol abuse. Using a trauma-informed lens, the case management services will facilitate wellness and recovery, contributing to an improved quality of life for individuals, families, and the community.
FACS is committed to helping individuals and families achieve wellness and sustain their recovery. As a Wellness and Recovery Support Case Manager, you will work in-home and in-community, making a positive impact in the lives of those who need it most by:

Providing case management services to individuals and families recovering from substance and alcohol abuse

Increasing clients' understanding of prevention, treatment, and recovery processes

Making referrals and link clients to individual and family-strengthening community resources

Offering support in life skills development, job readiness, parenting education, and coaching

Coordinating care that aligns with wellness and recovery goals, ensuring safe, timely, and client-centered services

Promoting clients' autonomy and long-term wellness and recovery

Engaging in group facilitation and public speaking when necessary

Maintaining accurate and timely records using an electronic health record system
QualificationsRequirements:

Bachelor's degree in social services or a related field

Minimum two years' experience providing case management services to children, adolescents, and families (exclusive of internships), including one year working with addiction related programs

Some experience facilitating or co-facilitating groups, or comfort with public speaking

Excellent communication skills, both verbal and written

Working knowledge of electronic health record systems

Bilingual in Spanish and English is required

Ability to work some evening and Saturday hours

Valid driver's license with a good driving record
