- Duration: 12 Weeks contract (Can extend)
- Weekly Gross Pay: $2,269.60
Requirements
- Full-time position with 40 guaranteed hours per week
- Monday–Friday schedule (8:00 AM–4:30 PM)
- No weekends
- No holiday shifts
- Skilled Nursing Facility setting

Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans based on functional goals and clinical findings.
- Provide evidence-based therapy interventions to improve strength, balance, mobility, endurance, gait, and overall functional independence.
- Perform comprehensive assessments and establish measurable rehabilitation goals.
- Monitor patient progress and adjust treatment plans as needed to maximize outcomes.
- Educate patients, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, and home exercise programs.
- Collaborate with physicians, nurses, occupational therapists, speech-language pathologists, and other healthcare professionals to coordinate patient care.
- Accurately document evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries in compliance with facility and regulatory standards.
- Recommend and train patients in the proper use of assistive devices, adaptive equipment, and mobility aids.
- Ensure compliance with Medicare guidelines, infection prevention protocols, and all applicable regulatory requirements.
- Promote patient safety while delivering compassionate, high-quality rehabilitation services.
- Maintain productivity standards while providing exceptional patient-centered care.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in the state of practice (or eligibility for licensure).
- Prior experience working in a Skilled Nursing Facility (SNF) is required.
- Strong clinical assessment, treatment planning, and documentation skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and collaboratively within an interdisciplinary healthcare team.
