Community PACE at Home is a Program of All-Inclusive Care for the Elderly (PACE) — a fully integrated, community-based care model designed to help frail older adults remain safely at home rather than moving to a nursing facility. PACE participants receive comprehensive medical, personal care, and social services coordinated by a dedicated Interdisciplinary Team (IDT).

Position Summary
Community PACE at Home is seeking a compassionate, reliable, and skilled Home Health Aide/Certified Nursing Assistant (HHA/CNA) to join our care team. Under the supervision of a Registered Nurse, the HHA/CNA will provide personal care services and supportive assistance to PACE participants in their homes and in the community. This full-time position offers steady, meaningful work with a consistent schedule and a supportive interdisciplinary team behind you every step of the way.
Key Responsibilities
Personal Care Services
- Assist participants with activities of daily living (ADLs) including bathing, grooming, dressing, oral hygiene, toileting, and continence care.
- Aid with transfers, ambulation, and mobility using proper body mechanics and assistive devices.
- Assist with meal preparation, feeding, and ensuring participants follow prescribed dietary guidelines.
- Perform light housekeeping tasks directly related to the participant's care, such as tidying the immediate living area and laundering personal items.
- Assist with medication reminders in accordance with the participant's plan of care and agency policy.
Observation & Reporting
- Observe and document participant's physical and emotional condition, reporting any changes in status promptly to the supervising RN or care team.
- Monitor for signs of skin breakdown, changes in cognition, fall risk, or other safety concerns and communicate findings to the clinical team.
- Maintain accurate and timely documentation of care provided in the electronic health record (EHR) or designated documentation system.
Participant & Family Support
- Build trusting, respectful relationships with participants and their families, honoring each individual's preferences, culture, and dignity.
- Provide companionship and emotional support, contributing to the participant's overall well-being and sense of connection.
- Communicate effectively with participants, family members, and the interdisciplinary care team regarding care needs and concerns.
- Reinforce care instructions provided by nurses, therapists, and other IDT members.
Safety & Compliance
- Follow all infection control, standard precautions, and safety protocols in the home and community setting.
- Adhere to Community PACE at Home policies, PACE regulatory requirements, HIPAA, and all applicable Michigan state regulations.
- Participate in required training, in-services, and continuing education programs.
- Report any incidents, accidents, or unusual occurrences immediately to the supervising nurse.
Qualifications & Requirements
Required
- Current, valid Michigan Certified Nursing Assistant (CNA) certification in good standing.
- Valid Michigan driver's license with a clean driving record.
- Reliable personal vehicle with current auto insurance.
- Current CPR/BLS certification (or willingness to obtain prior to start).
- Ability to perform physical demands of the role, including lifting, transferring, and assisting with mobility.
- Strong communication skills and a genuine compassion for working with frail elderly individuals.
- Dependable, punctual, and professional with a commitment to person-centered care.
Preferred
- Prior home health aide or home care experience preferred.
- Experience working with geriatric, medically complex, or memory care populations.
- Familiarity with electronic health records (EHR) or point-of-care documentation tools.
- Experience in adult day health, or long-term care setting.
Compensation & Benefits
- Competitive hourly wage of $17.00 – $19.00, based on experience and qualifications.
