Title:
Director of Nursing (RN) – East Side Cleveland Post-Acute / Skilled Nursing
Compensation:
$125,000–$135,000 annually (based on experience) + eligibility for DON bonus program
Location:
East Side Cleveland, OH (Lyndhurst)
Overview:
We’re partnering with a growing post-hospital rehabilitation and skilled nursing organization on the east side of Cleveland that offers a more modern experience than a traditional nursing home. The campus feature all-private suites and a hotel-inspired environment designed to support comfort, dignity, and healing for residents transitioning from hospital to home or living long term.
The organization is investing heavily in clinical leadership and quality improvement, seeking an experienced Director of Nursing (RN) to lead on Cleveland’s east side.
If you’re a hands-on nursing leader who loves building teams, driving survey readiness, and being visible on the floor — this is built for you.
What You’ll Do:
- Lead all aspects of the nursing department for a 100+ bed skilled nursing & rehab center with all-private resident suites.
- Partner closely with an engaged LNHA and corporate clinical leadership (CNO/CORP nursing team) to drive survey readiness, quality metrics, and QAPI.
- Build out and mentor a team of 4 nurse leaders (ADON/unit managers/night supervisor structure can be shaped with your input).
- Design and manage the call rotation so that leadership is present and supported without burning anyone out.
- Oversee clinical systems using PointClickCare (PCC) and ensure documentation supports excellent resident care and compliance.
- Round routinely, coach charge nurses and CNAs at the elbow, and stay highly visible to staff, residents, and families.
- Collaborate with therapy, social services, admissions/marketing, and providers to support smooth post-acute transitions.
- Lead and participate in QAPI, infection prevention, and ongoing improvement work with strong corporate support.
- Maintain compliance with all federal, state, and local regulations and respond confidently to surveyors.
Gorgeous Location – Post-Acute & Assisted Living Campus (Lyndhurst market)
- CMS 5-star Quality Measures rated
- Modern post-hospital rehab and skilled nursing center with private suites and an attached assisted living community.
- Recently welcomed into a major regional health system’s connected care network, creating strong hospital partnerships and clinical expectations.
- Newer but highly engaged LNHA who is passionate, collaborative, and looking for a strong clinical counterpart with deep survey and regulatory experience.
- Great opportunity for a seasoned DON who enjoys mentoring a newer administrator and shaping systems for the long term.
What’s In It for You:
- Competitive Salary: $125,000–$135,000 base, depending on experience.
- Bonus Program: Eligibility for an established DON bonus plan tied to organizational and clinical objectives.
- Health Benefits: Comprehensive medical, dental, and vision coverage with a large portion of healthcare premiums paid by the organization, often saving leaders an estimated $5,000–$8,000 per year compared to typical market contributions.
- Time Off: PTO package is negotiable based on experience and prior tenure.
- Eligibility: Benefits begin after 90 days of employment.
- Support: Direct access to a Chief Nursing Officer and corporate clinical/operations leaders who are present, responsive, and invested in your success.
- Impact: A chance to step into buildings with clear opportunities for growth and put your fingerprint on culture, systems, and outcomes.
What We’re Looking For:
- Current Registered Nurse (RN) license in Ohio (or compact eligibility with intent to practice in Ohio).
- 3+ years of nursing leadership in long-term care, post-acute rehab, or skilled nursing (DON, ADON, Unit Manager, Clinical Manager, etc.).
- Demonstrated experience with survey preparation, survey response, and quality-improvement work.
- Comfortable leading in a high-acuity, post-hospital rehab environment with medically complex residents.
- Strong team-builder who can coach, hold staff accountable, and still be seen as fair and approachable.
- Ability to collaborate effectively with physicians, advanced practice providers, therapy, and hospital partners.
- Experience with PointClickCare (PCC) or similar EMR preferred.
- Able to balance strategic thinking with “boots-on-the-ground” leadership — you’re as comfortable in QAPI as you are in a hallway or resident room.
