Job Summary
We are seeking a dedicated and compassionate LVN to join our healthcare team. The ideal candidate will provide high-quality patient care in the patient's home. This role requires a strong commitment to patient advocacy, effective communication skills, and the ability to work collaboratively with interdisciplinary teams. LVN field nurses are responsible for daily home visits, assessing patient needs, and ensuring the delivery of appropriate medical interventions.
As a LVN, you will:
- Work one-on-one with patients in a home environment
- Computerized documentation
- Exercise strong decision making and judgment skills

Skills
- Proficient in performing vital signs monitoring and laboratory assessments.
- Strong knowledge of coding practices related to patient care documentation.
- Experience in outpatient settings preferred, with a focus on patient-centered care.
- Excellent interpersonal skills for effective communication with patients, families, and team members.
- Ability to work under pressure while maintaining attention to detail in a fast-paced environment.
- Familiarity with discharge planning processes to facilitate optimal patient outcomes.
- Commitment to ongoing professional development in nursing practices and healthcare trends.
Qualifications:
- Must be licensed in the State of Texas as a LVN.
- Diploma, Associate, or Baccalaureate degree in nursing
- Minimum of one year Home Healthcare experience preferred (but not required).
- Must work well independently, enjoy problem solving and understand the importance of accurate documentation.
- Thrives in situations requiring critical thinking.
- Valid Driver’s license and proof of automobile insurance.

Pay: $55,000.00 - $73,000.00 per year
