Position Overview:
The Case Manager provides comprehensive, person-centered services to individuals with substance use disorders, primarily participants of the STAR (Support Team for Addiction Recovery) Program. Based in our Newton office with community-based outreach, this role supports individuals in addressing barriers to recovery and improving overall quality of life through coordinated access to treatment, healthcare, housing, employment, education, and other support services. Working collaboratively with a Peer Recovery Specialist and other service providers, the Case Manager helps participants develop and achieve recovery goals while promoting self-sufficiency, wellness, and long-term recovery.
Qualifications
- Bachelor’s degree with CADC or LCADC credential, or master's degree in health, psychology, counseling, social work, education, or other behavioral health profession, required.
- Minimum of three years of experience working with individuals with substance use disorders or other high-need populations.
- Demonstrated knowledge of substance use disorders, recovery principles, and community-based treatment resources.
- Knowledge of formal and informal community systems, service networks, and referral resources.
- Ability to build rapport and effectively engage individuals from diverse backgrounds.
- Excellent interpersonal communication, documentation, organizational, and computer skills.
- Ability to work both independently and collaboratively as part of a multidisciplinary team.
- Must possess a valid NJ driver's license in good standing, maintain a safe driving record, carry adequate automobile insurance, and have reliable transportation.
- Ability to travel throughout the community to meet with clients and community partners.
- Flexibility to work occasional evenings or weekends as needed.
- Bilingual English/Spanish skills are a plus.
Responsibilities:
- Conduct comprehensive case management assessments addressing housing, finances, transportation, legal services, employment, education, healthcare, and family strengths and needs.
- Provide weekly in-person and telephone outreach and support sessions.
- Use motivational engagement strategies to encourage voluntary participation in services and recovery supports.
- Advocate on behalf of individuals to help remove barriers and improve access to resources.
- Maintain timely and accurate documentation in accordance with agency and grant requirements.
- Participate in team meetings, supervision, and trainings.
- Perform other duties as necessary or assigned.
Position Details:
This is a full-time, 40-hour per week position. Flexibility to work occasionally evenings and weekends as needed.
Compensation & Benefits:
· Annual salary: $53,000 - $55,000, commensurate with experience.
