The Individual Needs Assessor at JCOA Main works in the Care Management Department and is responsible for assessing seniors age 60 and older to determine their eligibility for Jefferson Council on Aging services. The role involves completing Louisiana Independent Living Assessments and/or NFCSP Assessments, including initial in-home assessments after referral and annual reassessments. Key duties include calling participants, meeting with seniors and/or families in their homes, collecting signatures, entering information into JCOA's database, submitting completed assessments every two weeks, and attending trainings or meetings as needed. The position requires a 2-year degree or four years of relevant experience, strong communication skills, confidentiality, attention to detail, the ability to work independently, and comfort interacting with elderly individuals from diverse backgrounds. The job has no supervisory responsibilities, is non-exempt, and pays $12.00 per hour.
