Responsibilities
Provide direct patient care to residents in our skilled nursing facility, including medication administration, wound care, and vital sign monitoring
Collaborate with other healthcare professionals to develop and implement individualized care plans
Conduct vital sign checks and document resident data accurately
Report changes in resident condition to the registered nurse or physician
Participate in quality improvement initiatives to enhance the overall resident experience
Requirements
Current licensure as a Licensed Practical Nurse (LPN) in the state of Oklahoma
High school diploma or equivalent required; associate's degree or higher in nursing or a related field preferred
