About the Licensed Practical Nurse (LPN) Role:
We are seeking a compassionate and skilled LPN to join our Home Health team in Philadelphia, Pennsylvania. As an LPN with Deer Meadows Home Health, you will provide direct patient care, administer medications, and educate patients and their families on disease management and self-care techniques. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, ensuring that patients receive the best possible care and support.
Licensed Practical Nurse (LPN) Responsibilities:
Provide direct patient care, including vital sign assessments, wound care, and medication administration
Develop and implement individualized care plans in collaboration with the healthcare team
Educate patients and their families on disease management, self-care techniques, and medication management
Document patient progress and care provided in accordance with regulatory requirements
Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Maintain accurate and up-to-date patient records
Participate in staff development and continuing education programs to stay current with industry best practices
Licensed Practical Nurse (LPN) Requirements:
Current LPN licensure in the state of Pennsylvania
1+ years of experience as an LPN in home health or a related setting
Strong clinical skills and knowledge of patient care principles
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Reliable transportation and a valid driver's license
